The following items are typically needed for FDA/MQSA inspections (click on Red file-link for document)

1. Initial MQSA qualifications (all in one document) – MQSA qualifications

**PLEASE NOTE**:   The MDH Service Provider certificate provided with your report (usually on last page) is the one you need for inspections.  That certificate was valid at the time we were on-site and you do do not need to request an updated copy because the date range may not match the survey date.  The inspector will want to see the certificate that was valid at the time I was on site for the inspection.
